Is this for US citizens returning to the US?! I thought that was long gone . . .
DebD, did you have to upload a picture of your vaccination card or just swear that you are vaccinated? When we returned home from Italy last July we did not have to fill anything like this out.

I think you checked the wrong box along the way. You don't need to do this.
My guess is this is a glitch in the system when one asks for boarding passes prior to checking in? It really makes no sense on a few levels otherwise. Just guessing that something was not updated post covid/testing rules as it should have been via Jet Blue's system?
